PRICING AND PAYMENT INFORMATION

We are a fee-for-service practice and fees are due at time of service, no exceptions.  We do not accept insurance (commercial or Medicaid) with the exception of commercial Humana after May 1, 2022.  If you would like to file a claim for your visit with your insurance company you will be able to print out a superbill from the patient portal after each visit for reimbursement directly from your insurance company; fees will need to be paid at the start of each visit.

Self-Pay Fee Schedule/Pricing for Cheryl Marrs DNP, APRN, PMHNP-BC:

•  INITIAL EVALUATION (approximately 60-90 minutes):   $300

• FOLLOW-UP/MEDICATION CHECK (approximately 5-20 minutes: $75; 21-30 minutes: $100; 31-40 minutes: $125; 41-50 minutes: $150).  The amount of time you need must be scheduled at the time of booking as I will not take away from another patient's allotted time.  If you did not schedule yourself enough time you will need to schedule another appointment.  The fee is calculated based upon actual visit time, not amount of time scheduled by the patient.

• THERAPY ONLY (approximately 30 minutes: $100; 50-60 minutes $175); Family or Couples Counseling:  (50-60 minutes $200}

• There may be a fee associated with medication refills requested outside of your office visit, especially if this is a repetitive request.  

• No-show appointments or appointments canceled with less than 24 hours notice will be charged at the same amount as the appointment fee and will need to be paid prior to the next appointment.  Excessive no-shows/late cancellations are cause for termination from the practice; if an initial evaluation is a no-show without cause, there is a 6 month waiting period before another new patient appointment can be made.

ADDITIONAL SERVICES

We may also offer additional services billed at a rate of $30 for every 15-minute increment--these are NOT covered by insurance and will need to be paid for up front. These services include, but are not limited to:

• Letter writing

• Care coordination

• FMLA

• Emotional Support Animal

• Accommodation letters/forms

This is ultimately up to the provider to decide if she is able to furnish this service; she has no obligation to supply it.

CANCELLATION POLICY

You will be charged the full appointment fee for all scheduled services that are missed (no call/no show) or cancelled with less than 24 hours notice.

This is an outpatient psychiatric practice. We do not provide crisis services, after hours consultation, after hours or weekend medication refills, or on-call services. If you have a psychiatric emergency:  call 911 or go to your nearest emergency department.
