Please make sure you look at the pricing page prior to scheduling your appointment so you have prior knowledge of the cost. It is very important when you request an appointment that you look for a confirmation email from the office to see if your request was approved; it's not on the schedule without confirmation (otherwise you will get a denial email stating the reason). If the appointment was approved, you will need to open the confirmation email and follow the link to the patient portal to the consent forms. These need to be filled out no later than 48 hours prior to your scheduled day/time; if this is not done your appointment will have to be rescheduled.
